Please ask at Reception to see if there are currently any spaces left on our patient list. All new permanent patients will be required to undertake a new patient medical within 6 months of joining us. The GPā€™s here run a personalized list so upon registering you will in most cases see your own GP each time you attend surgery. This makes for excellent continuity of care and an enhanced GP/Patient relationship. New patients whether temporary or permanent will be required to show two pieces of proof of identity and permanent address. A combination of two of the following will usually be requested as suitable forms of ID ā€“ one including photographic ID and another as proof of address

  • Passport
  • Driving Licence
  • Current paid utility bill
  • Local authority rent card/Private rental agreement document
  • Marriage/birth certificate
  • Papers from the Home Office or UK Border Agency
  • Bank/Building Society card/statements

Patient’s who would like to register with us should attend the surgery after 10am only to complete the relevant paperwork. Before this time the admin staff are busy booking emergency “on the day” appointments. Many thanks
