Registrations are now done via appointment only –  do not come down to surgery if you have not been given a time to come and complete the relevant paperwork – YOU MUST BE WITHIN OUR BOUNDARY AREA TO REGISTER – PLEASE CHECK YOUR POSTCODE ON THE HOMESCREEN OF THIS WEBSITE

Please ask at Reception to see if there are currently any spaces left on our patient list. All new permanent patients will be required to undertake a new patient medical within 6 months of joining us. The GP’s here run a personalized list so upon registering you will in most cases see your own GP each time you attend surgery. This makes for excellent continuity of care and an enhanced GP/Patient relationship. If possible we would like new patients whether temporary or permanent, to show two pieces of proof of identity and permanent address. A combination of two of the following will usually be requested as suitable forms of ID – one including photographic ID and another as proof of address

  • Passport
  • Driving Licence
  • Current paid utility bill
  • Local authority rent card/Private rental agreement document
  • Marriage/birth certificate
  • Papers from the Home Office or UK Border Agency
  • Bank/Building Society card/statements

If you do not have any identification this will not prevent you registering at the surgery.

Catchment Area

To check to see if you are living inside our Practice Boundary please look at the home screen catchment area postcode checker. We can only register new patients if they are living in our boundary area – any current patient who moves out of the area will be required to register with a new GP and we will remove them from our list in 28 days of their home address being updated on our system. We make no exceptions to this rule.
